Clinic-Level Care, Delivered to Your Living Room

An in-home physical therapist on Long Island does everything a therapist in an outpatient clinic does — a full evaluation, hands-on manual therapy, a progressive exercise program, weekly reassessment — except the visit happens at your home. At Clarke Care Physical Therapy, that isn’t a side offering. It’s the entire practice.
We bring what the session needs with us: manual therapy and joint mobilization, cupping, Rock Blade instrument-assisted soft-tissue work, kinesiology taping, and an exercise program tailored to the space and equipment you actually have. If your only stairs are the four concrete steps off the back deck, those are the stairs we train on.
Our team is based in Freeport and travels across Nassau and Suffolk. We treat every age, from infants through seniors, and every stage — post-surgical, chronic, neurological, athletic, prenatal and postpartum.

Why Long Islanders Choose Home Visits

The Commute is a Big Reason People Quit PT

Most patients don’t stop because therapy stopped working. They stop because a 45-minute session turned into a two-hour errand — and after three or four weeks, that math stops making sense.

Distance

Nassau and Suffolk are spread out.
Between the Southern State, the LIE, and school pickup, a clinic ten miles away can eat your whole afternoon. Your therapist absorbing the drive instead of you is the difference between finishing a plan of care and abandoning it.

Post-Surgical

You shouldn’t be driving yet.
After a knee replacement, rotator cuff repair, or spinal surgery, getting in and out of a car is often the hardest and riskiest thing you’ll do that day. In-home care removes that trip entirely during the weeks it matters most.

Fall Risk

Balance work belongs at home.
Fall prevention is only as good as the environment it’s practiced in. We assess your actual thresholds, rugs, stair rails, and bathroom — then train balance and gait against the real hazards instead of a clinic’s flat, uncluttered floor.

Families

Caregivers can be in the room.
A spouse, adult child, or parent can watch the session, learn the transfers and cues, and ask questions in real time. That’s difficult to arrange in a clinic, and it changes how well a home program actually gets followed.

Pediatrics

Kids do better on home turf.
A familiar room, familiar toys, and a parent nearby produce a far more accurate picture of a child’s movement than an unfamiliar clinic does — and a far more cooperative session.

Winter

Weather stops being a factor.
January on the Island means ice, cancellations, and gaps in care right when stiffness and joint pain are at their worst. Home visits keep the plan on schedule through the months that usually derail it.

We continuously track your progress, progress your plan weekly, and equip you with a clear home exercise program so you finish care with the knowledge and skills to maintain your results long-term.

Frequently Asked Questions About In-Home Physical Therapy on Long Island

Do I need a doctor's referral for in-home physical therapy in New York?

Usually not. New York allows direct access, so most patients can begin physical therapy without a physician’s referral. Some insurance plans still require one — we check your plan when we verify your benefits and tell you right away if a referral applies to you.

Does insurance cover an in-home physical therapist?

In most cases, yes. Clarke Care accepts most major commercial insurance plans, Medicare, and Medicare Advantage, and we handle benefits verification for you. Patients typically pay the same co-pay their plan requires for a clinic visit.

Is this the same as home health care?

Not quite. Traditional home health typically requires that you be certified as homebound and often bundles nursing, aides, and therapy under one agency plan. We provide outpatient-level physical therapy delivered at home — you don’t need to be homebound to qualify, and the visits are longer and more hands-on than a standard agency therapy visit.

What towns on Long Island do you cover?

We’re based in Freeport and travel throughout Nassau and Suffolk County, plus neighboring parts of Queens. Coverage in eastern Suffolk depends on scheduling — call (516) 559-6751, and we’ll tell you honestly whether we can reach you.

Do I need equipment or a lot of space at home?

No. Your therapist brings what the session requires, and the program is designed around the space you have. A hallway, a sturdy chair, and a few feet of clear floor are enough for most plans. Working with your real furniture and stairs is an advantage, not a compromise.

How long is a visit, and how often will you come?

The initial evaluation runs 60–75 minutes. Follow-up visits are typically shorter and scheduled one to three times per week depending on your condition, your surgeon’s protocol if you have one, and how you respond. We reassess and adjust weekly.

Who can receive in-home physical therapy?

Patients of every age, from infants through seniors. Home visits are a particularly strong fit for post-surgical patients, older adults managing fall risk, children who do better in familiar surroundings, new and expecting mothers, people with limited mobility, and anyone whose work hours make clinic appointments unrealistic.
